Continuum of Care
Inpatient rehabilitation is only the first step of the rehabilitation journey. Our team will refer you to follow-up services – perhaps home health, home and community rehabilitation or outpatient therapies. Follow-up clinics with our rehabilitation physicians are offered to those patients who need it.
